Credential Verification Fee
The American Board of General Dentistry has initiated a handling fee for credential verifications. The fee is $15.00 per verification per individual. Payments can be made by check, money order or credit card (MasterCard or Visa only).
Please send a written request, signed release from the dentist and the handling fee to the American Board of General Dentistry, 211 E. Chicago Ave, Suite 900, Chicago, IL, 60611-1999.
Verification requests with credit card payments may be faxed to (312) 440-0559.
No credential verifications will be given out over the phone or through e-mail. For questions please contact :
